The key insight is that not all nodes are equally important to the user. Some are trivial and can be logged silently; others are critical and require explicit transparency.
Conducting the Audit
To conduct a Decision Node Audit, follow these steps:
- Map the workflow – List every step the agent takes, from input to output, including sub-steps.
- Identify decision nodes – Highlight points where the agent chooses between options or applies a probability.

Initial Black Box Failure
Consider Meridian (not real name), an insurance company that uses an agentic AI to process initial accident claims. The user uploads photos of vehicle damage and the police report. The agent then disappears for a minute before returning with a risk assessment and a proposed payout range.
Initially, Meridian’s interface simply showed “Calculating Claim Status.” Users grew frustrated. They had submitted several detailed documents and felt uncertain about whether the AI had even reviewed the police report, which contained mitigating circumstances. The Black Box created distrust.

Prioritizing with an Impact/Risk Matrix
The Impact/Risk Matrix helps you decide which decision nodes to expose. Plot each node on two axes:
- Impact on outcome – How much does this node affect the final result? (Low to High)
- Risk of error – How likely is the AI to make a mistake at this point? (Low to High)
Nodes in the high-impact/high-risk quadrant (top-right) demand the most transparency—typically an Intent Preview or explicit confirmation. Low-impact/low-risk nodes can be handled with a simple log entry or even left silent. Nodes on the diagonal require careful judgment; a subtle indicator (e.g., a changing icon) might suffice.
